ANPAC® offers benefits and
services that account for approximately 39% over and above each
Team Member's base salary. This includes:
Group Health Plan (includes Major Medical, Short-term Disability,
Term Life, and Accidental Death & Dismemberment):
Major
Medical provides benefits for covered expenses resulting from an
illness or non-work related injury. Coverage is also provided for
eligible mammograms, bone mass measurement, prostate cancer screenings,
childhood immunizations, etc. Major Medical includes a Prescription
Card Service, which allows 30-day prescriptions to be filled for
a $10 co-payment for a generic drug (or $20 co-payment for a brand
name drug with no generic equivalent.) There is also a Prescription
Home Delivery Service, which offers at-home delivery of prescriptions
(up to a 90-day supply for a $10 co-payment for a generic drug or
a brand-name drug with no generic equivalent). Dental coverage is
not part of this plan, except for certain listed services (see plan
booklet for details.)
Short-term
Disability* is part of the Group Health Plan and pays 66-2/3% of
a covered Team Member's weekly salary up to 24 weeks should they
become disabled (after 6 months of service) and not have enough
paid sick leave to cover the absence. STD coverage (without enrolling
in Major Medical) is $6.40/month.
Term
Life Insurance and Accidental Death & Dismemberment plans are
paid by ANPAC and are equal to twice the Team Member's annual salary,
up to $100,000.
*ANPAC® pays approximately
70% of the Major Medical & Short-term Disability premiums and
100% of the Term Life and Accidental Death & Dismemberment premiums.
Additional
Insurance:
After
2 years of service, Team Members are automatically covered under
American National's Long Term Disability plan. LTD is fully paid
by ANPAC and coverage includes 60% of the average weekly earnings.
LTD begins 6 months from the date of disability and continues to
pay up to age 65 or longer (determined by the policy provision under
Benefit Duration Limit), provided that the Team Member continues
to be disabled.
Voluntary
Group Term Life (individual or family policy) is available upon
hire or during annual enrollment. The rates are age-banded and coverage
may be 1, 2, or 3 x annual salary. Voluntary Accidental Death &
Dismemberment (individual or family policy) is also available upon
hire or annual enrollment. An insurance plan available after 1 year
of service, LPU@96 (a whole life policy that builds cash value)
may be purchased directly from our parent company with no agent
commission or load fee charged.
Discounted
Auto Insurance through ANPAC's BenefitsXpress program is available
for those Team Members who have their personal automobile(s) and
home insured by ANPAC®.
Dental
and Vision:
There
is a Voluntary Vision Insurance Plan and a Voluntary Dental Insurance
Plan available to ANPAC Team Members.
Flexible
Spending Accounts (FSA):
There
are two types of Flexible Spending Accounts: Health Care Account
and Dependent Care Account. These accounts allow you to pay health
care expenses (not covered by a health insurance plan), or dependent
care expenses with tax-free dollars. You may participate in either
or both accounts through payroll deduction. You decide each year
how much you want to deposit to each account. The minimum amount
you may contribute to either account is $240 per year ($10 per pay
period). The maximum you may contribute in 2005 is $3600 ($150 per
pay period) in the Health Care FSA and $5000 ($208.33 per pay period)
in the Dependent Care FSA. Your contributions must be used for expenses
incurred in the same year.
Retirement:
There
is a Retirement Plan fully funded by ANPAC®.
Team Members, at least 21 years of age, begin participating in the
plan after 1 year of service and are fully vested after 5 years.
It is a "defined" plan and is based upon years of service
and highest 5 years of earnings.
The
401(k) Plan is an option Team Members have to personally supplement
their retirement income benefits. Deductions are pre-taxed, and
participants choose from a variety of investment funds. Contributions
can be as little as $10 per pay period, up to a $15,500 annual maximum
(as prescribed by law).
Time-off
Benefits:
Vacation is awarded on January 1 each year, even though it is earned
throughout the year, and includes 10 days for the first 4 years
and one additional day for each continuous year, up to 25 days.
Vacation can be taken in ½ or full-day increments and does
not carry over from year to year. During first year of employment,
vacation accrues at a rate of 1 day/month, after completing the
6-month Introductory Period.
ANPAC®
offers 3 Floating Holidays per year that may be taken for holidays
that a Team Member observes but the company does not or for emergency
situations that cannot be scheduled in advance (i.e., sick child).
Days may be taken in ½ or full-day increments. After the
6-month Introductory Period is met, floating holidays accrue at
the rate of 1 day per calendar quarter. ANPAC®
observes 8 Holidays throughout the year: New Year's Day, Memorial
Day, Independence Day, Labor Day, Thanksgiving Day, Day after Thanksgiving,
Christmas Eve, and Christmas Day.
Sick
Leave may be used for times when Team Members are unable to work
due to illness or injury. Team Members accrue 5 days per year which
can carry over from year to year, up to a maximum of 90 days. After
5 years of service, Team Members earn an extra sick day per year,
up to a maximum of 10 per year. The accrual rate for new hires is
1/2 day per month, after the 6-month Introductory Period is met.
Job
Posting and Educational Assistance:
ANPAC®
has a Job Posting program and promotes within as much as possible.
This allows opportunities to Team Members who prove themselves to
be professionally effective in their positions. ANPAC®
posts job openings for qualified Team Members to be considered for.
To assist in advancement, ANPAC®
encourages continuation of education/training. The Education Program
pays 100% (up front) for job-related courses.
